OOPS! Withdraw my question . I was reading the notes in the wrong order;
this issue has already been raised.

PT, it looks like this was published in 1994. Is there a more current
version out there? (There have been a lot of UI changes since then.)

Microsoft published a book with a title something like "The Windows
Interface Guidelines for Software Design" available on Amazon at
<http://www.amazon.com/New-Windows-Interface-Microsoft-Corporation/dp/155615
6790>
http://www.amazon.com/New-Windows-Interface-Microsoft-Corporation/dp/1556156
790.
I don't have it here with me, but it is also available from Microsoft Press.
I use it extensively when I am testing, writing requirements, and reviewing
software in development for usabilty. It has great pictures to show the
developers ;-D
It's a great tool for tech writing because of the glossary and good
annotated graphics (for "what is THAT called" moments).
It has everything you could ever want, from button placement to windows
titles, to--yes--forms.
Hope this helps.
